Vital Shield 2900 (Vital Shield) is a lower priced PPO plan that offers you affordable coverage that protects you in case of major medical events, such as hospitalization. How it works
1. You pay the first $2,900 in medical bills (you still have preventive benefits listed below).
2. Once you meet the $2900, you pay 40% of Blue Shield's contracted rate for services up to $5,900. (Exception: You pay 100% of negotiated rate on lab & X-ray until copayment maximum is reached.
3. Once you meet $5,900 copayment maximum you are covered 100%.

Preventive benefits
Prior to meeting your annual deductible you get:

• Two office visits per year (preventive, office or both).
• $10 generic drugs.
• 20% off vision related products and services.

Access to a large PPO Network
When you enroll in Vital Shield, you'll have access to Blue Shieldıs large network of physicians and hospitals.

Dental Benefits
Taking care of your teeth can help you look and feel better. You can add a comprehensive HMO or PPO dental plan and still keep your total premium low!

Dental PPO ($35 individual, $70 two-party, $110 family)

• An individual annual deductible of $50 per member
• A wide range of dental benefits, including diagnostic and preventive services at no charge
• A $1,000 per member per year benefit maximum
• No waiting period for diagnostic or preventive services

Dental HMO ($17 individual, $34 two-party, $55 family)

• No deductibles and no calendar year maximums 
• A wide range of dental benefits, including diagnostic and preventive services at no charge 
• No waiting period for most services (There is a 12-month waiting period for orthodontic services.)
