Blue Shield Essential Plan — One Plan, Two Options
There are two Essential Plan packages — Essential
Plan 3000 and Essential Plan 4500.
Both plans include the same great
coverage: medical and hospital services, dental care, and
vision care. Which package is for you? The difference lies in
the deductible.
Essential Plan 3000 has a deductible of $3,000,
which keeps monthly premiums low. Essential Plan 4500 has
a deductible of $4,500, so monthly premiums are even lower — as
low as $77.
So what do you get?
- Three office visits a year — only $40 each
- Preventative care (including an annual exam) — $40 a
visit
- Emergency health coverage — $100
- Dental exams, cleanings, and x-rays — free
- Vision exams — $5
- Generic prescription drug coverage — $10/prescription
And, you can get a 15% discount on LASIK corrective
eye surgery.
