Option Twelve is a Preferred Provider Organization (PPO) plan. That means it works with a managed care "network" of healthcare providers. Blue Shield has one of the largest networks in California, with over 45,000 doctors and hospitals to choose from.

The care providers offer their medical services at a discount, which means you'll have affordable monthly premiums and co-payments.

My Coverage

Your coverage with the Option Twelve plan includes protection for:

• Laboratory exams

• Prescription drugs

My Deductible

You'll choose a $500, $1,000, $1,500, $2,000 or $3,000 deductible, so you can customize your Option Twelve plan to fit your budget.

A lower deductible doesnęt mean you'll save money. The higher deductible you choose, the lower your monthly premiums will be set. But you don't want to get a deductible that's too high, either. Usually it's best to choose the highest deductible that you can realistically afford.
